WHAT IS PACE CARE?

PACE, a Program of All-Inclusive Care for the Elderly, is designed to help older adults with complex physical or medical needs remain living independently in their homes and communities.

PACE provides older individuals, their family, caregivers, and professional healthcare providers with the flexibility to meet the participant’s needs in the community.

Medical Care : Primary care, specialist visits, and care coordination

Day Center Services : Meals, social activities, and wellness programs

Home Care : Assistance with daily activities at home

Therapies : Physical, occupational therapy as needed

Transportation : Rides to and from medical appointments and the PACE center

Medications & Supplies : Prescription drugs and durable medical equipment

Eligibility

To enroll in K-day, you need to:

Be 55+ years
Reside in K-day’s service area(check below zip code)
Require nursing facility level of care, as defined by California
Be able to live safely in the community with the help of PACE services at the time you enroll.
